The Underworld is the 5th dungeon in Dungeon Quest, added on 2019-04-28. The Underworld requires players to be level 80 for Insane difficulty and level 85 for Nightmare difficulty. The theme of this dungeon is hell where demons reside.
Mini-bosses include the Demonic Overgrowth in the 4th room and Kolvumar in the 8th room. The final boss is Demon Lord Azrallik, who resides in the 10th room. This dungeon features a normal melee enemy, a normal ranged enemy, a summoned melee enemy, and a special ranged enemy with an attack that tracks the player. It is recommended to have at least 40K+ health for Insane difficulty and 65K+ health for Nightmare. Players have 10 minutes to clear the dungeon on Insane and eleven minutes on Nightmare.
The main theme is (raw) Fight Fight Fight A.
Mobs
In The Underworld, there are four different types of mobs. Each mob has around 14.38M base health and can deal 9.5K damage on Insane difficulty. They also have around 38.86M base health and can deal 10K damage on Nightmare difficulty.
Demon Warrior
The Demon Warrior is a melee-type mob. The Demon Warrior is a humanoid being whose entire body is covered red, having two ripped wings, and has a glowing aura demon sword. They are fully physical and have normal walkspeed, and will only deal damage when they get close to a player.
Dark Mage
The Dark Mage is a ranged-type mob. It is a humanoid demon whose entire body is black, equipped with a gray staff, and has two black horns. They are fully ranged and have slow walkspeed. They will keep shooting even if there are no nearby players if they have been aggravated. The Dark Mage will aim a red rectangle with a large width and long range on the ground in the direction they are facing. Their spell attack fires quickly after being aimed, causing a row of spikes to shoot out of the ground within the area of the attack. There is a short cooldown before they can fire again.
Elder Dark Mage
The Elder Dark Mage is a range-type mob. It is a large translucent red humanoid demon, having two ripped wings, a staff, and is immobile. The Elder Dark Mage attacks by summoning an AoE circle below a random player within its range, regardless of damage or taunts. The targeted player can dodge this attack by moving in a straight line. After firing at a target, there is a cooldown before they can fire again; it is longer than the Dark Mage's cooldown.
Blood Minion
Blood Minions is a melee-type mob. It is a small crimson humanoid being made from blood. are small crimson humanoid melee enemies. The Blood Minion can be only summoned by both the Demonic Overgrowth and Demon Lord Azrallik. They have 40% of the health that normal Mobs have and cannot be taunted. If there are three present on Overgrowth or five present on Azrallik, the Minions will become enraged when another attempts to spawn, as this is the most that can be spawned at one time. When enraged, they start to glow red, their walkspeed will greatly increased, and their damage is doubled. When each Minion is defeated, more will continue to spawn until the host (Overgrowth or Azrallik's heart) is defeated.
Bosses
The Underworld has three bosses, consisting of two mini-bosses and a main boss at the end of the dungeon. Each mini boss has a base health of 41.42M on Insane, and 196.9M on Nightmare difficulty. While the main boss has a base health of 56.96M on Insane and 221.51M on Nightmare difficulty.
Demonic Overgrowth
The Demonic Overgrowth is the first mini-boss the player will encounter in The Underworld, located in the 4th room. This boss has the appearance of a large mole protruding from the floor. The encounter begins when a player crosses the second line on the floor.
Demonic Ground Spikes: The Overgrowth starts out by targeting a random player with a long-range spike attack. The coverage of these Spikes is wide and activate quickly after targeting a player. They will aim three sets of Ground Spikes before continuing onto the next attack.
Spike Rows: Next, The Overgrowth summons a wide set of alternating Spike Rows that fire across the room. The rows are as wide as the area between each Spike. This attack fires at the same speed as the previous targeting attack.
Summoning Blood Minions: When the attack cycle repeats, The Overgrowth will summon a Blood Minion. Minions spawned by the Overgrowth deal 4k damage on Insane and 6k damage on Nightmare, and it is possible for them to deal two hits at the same. If there are three Blood Minions present before Overgrowth is defeated when the Overgrowth tries to summon another. all existing Blood Minions will be enraged, dealing double damage, and their walkspeed increases.
The Demonic Overgrowth base health is 41.42M on Insane and 196.9M on Nightmare difficulty. Each attack does around 8.33K on Insane and around 16.875K on Nightmare difficulty.
Kolvumar
Kolvumar is the second mini-boss the player will encounter in The Underworld, located in the 8th room. Its appearance is a large moose with a pair of human hands extending out of its face, having its entire body being dark red. The encounter begins when a player comes within its range, around a quarter of the way through the room. Kolvumar has a total of two moves.
Fire Circles: When Kolvumar is encountered, a random player is targeted with a large circle of fire, which starts to cause damage after two seconds, and continues to deal one tick of AoE damage every second until it is defeated or the attack fades. For every attack cycle after the first, Kolvumar will target two Fire Circles each time. As the fight continues, the time between each circle's summoning gradually decreases until they eventually appear at the same time.
Charge: After using its Fire Circles, Kolvumar then proceeds to run toward a random player, leaving a trail of fire that deals one tick of AoE damage every second until it is defeated or the attack fades. As the fight continues, Kolvumar will gradually Charge faster.
Kolvumar has a base health of 41.42M on Insane, and 196.9M on Nightmare difficulty. Each attack does around 11.66K on Insane, and around 19.375K on Nightmare difficulty.
Demon Lord Azrallik
Demon Lord Azrallik is the main boss in The Underworld. Demon Lord Azrallik is a very large humanoid being. Having his whole entire body dark red, his eyes are neon red, having four horns, and two things between the shoulder. He stands withing the void at the end of the room. The encounter begins after a player walks past the second row of light fixtures at the edges of the platform This boss has a total of four moves (one being temporary passive).
Demonic Lines: Azrallik's first attack sends an alternating row of lines away from him and toward the opposite end of the room, similar to that of the Demonic Overgrowth. When a line appears, it flashes and deals damage after one second.
Sunbeam Smash: Azrallik then smashes his left hand onto the ground, which causes a sunbeam of lines to shoot out from where he hit. These lines deal damage quickly after striking the ground.
Global Summoning: Azrallik's third attack summons a small AoE circle below every alive player, regardless of where they are. This attack pulses twice, with the second pulse doubling the circle's diameter. Each pulse takes one second.
Heart Attack [Passive]: When Azrallik reaches half health and is not instantly defeated, his main attacks will stop, and he seizes up, becoming immune to damage. He then proceeds to throw his heart onto the ground and falls backwards into the void below. The Heart will have 60% of his total health. When his heart hits the ground, a Blood Minion will spawn. Blood Minions spawned by the Heart deal 5K damage on Insane and 8K damage on Nightmare difficulty. Blood Minions will continue to spawn every two seconds until there are five Blood Minions present or until the heart has been defeated. Once all five Blood Minions are present, no more are able to spawn. And if the heart tries to summon another Blood Minions, all existing Blood Minions will be enraged, dealing double damage, and their walkspeed increases. After the heart is defeated, Azrallik will reappear from the void and return it to his body. This attack cannot be used again, and he will repeat using his main moves.
Demon Lord Azrallik's theme is (raw) Epic Trailer. Azrallik has a base health of 56.96M on Insane, and 221.51M on Nightmare difficulty. And each attack does around 12.5K on Insane, and around 21.25K on Nightmare difficulty.
Rooms
The spawn is located at the entrance of a large ajar door overlooking a staircase and a large open area, with other rooms visible in the distance over a dark void. This area opens up into a corridor with black pillars on its side. This corridor leads to a large circular room with four spikes in an arc extending from the corners and far above the room. The far left spike as seen from the entrance of this room contains an Egg. Another corridor sharing the same design as the first continues off to the right. This leads to a large room where the Demonic Overgrowth sits, blocking the opposing doorway. After it is defeated, this doorway opens up to another hallway. This leads to another circular room with the same design as the previous. The next corridor continues straight, leading to the mouth of a dark cave. This cave is blocked off by the Kolvumar. After defeating it, a doorway opens up to a larger path that makes its way into a Colosseum. This Colosseum houses Demon Lord Azrallik, a large creature that stands within the void at its center and overlooks the entrance.

Gregg
Gregg is a boss with a low spawning chance that players can choose to fight once per dungeon run in exchange for loot drops, gold and between 1-3 crafting materials used to craft powerful, permanent Legendary weapons near the Blacksmith in the Lobby. The crafting material dropped by Gregg in The Underworld is Warforged Metal.

Drops
Upon completing the dungeon, players receive a random item (Two items if on Hardcore mode) and gold, similarly to other dungeons excluding event ones. For Wave Defence, one item is received after every 15 waves. Players with the Extra Item game pass receive receive an extra item in addition to the above. Trivia
- This, Samurai Palace, Gilded Skies and Yokai Peak have the most rooms (10) among every dungeon.
- The update with The Underworld introduced the Cosmetic shop, a rotational shop where players can buy item skins and enchants with Robux.
- This is the first dungeon to have an immobilized mob, being the Elder Dark Mage.
- The questing theme hasn't played for a long time due to copyright violations. Currently, the music was re-added.
- Before the Northern Lands update, this dungeon has been nerfed the most out of all dungeons. On Nightmare, minions originally had 65M HP, mini-bosses had 220M, and the boss had 240M. After a few days, minion health was nerfed to 60M.
- Before Samurai Palace it was nerfed to 55M then eventually down to 45M. Mini-bosses were nerfed only once, from 220M to 200M and the boss from 240M to 225M.
- As of the Enchanted Forest update, this dungeon was further nerfed to make the Demon Warriors slightly slower than the player.
- As of 08/29/2024 update, enemies HP were reduced by 20-40% on Insane difficulty and by 10% on Nightmare difficulty.

- The Draugr Warrior/Mage in Hela Boss Raids are basically a reskin of Demon Warrior and Dark Mage.
- Azrallikās Heart is the only entity to not have an abbreviated health bar.
